It is a blenny. Where was the photograph taken?

Yes, like ATJ said, it is a blenny. I doubt that you will be able to get anything more specific from the picture.
From the coral in the background, the fish was photographed somehwere in the pacific.

I think it might be Ecsenius namiyei which has varying combinations of blue stripes & dots on its head. There are several pics on this site (although I haven't figured out how to get to them all) and you'll find more with google image search.
