Seizure at depth sounds more like oxygen toxicity or carbon dioxide toxicity , may have gotten DCS superimposed when they surfaced with him.
Oxygen toxicity could have been caused by faulty oxygen sensors, however a more common cause of problems is Carbon dioxide buildup, which could also lead to seizures and death.
