This is a replacement kit (Aquatica part no. 18583) consisting of a BK-7 glass (not perspex) lens for Aquatica AF/MF flat macro ports 18426, 18428 and 18429. In addition to the lens, the kit comprises the required O-ring, a 1/16th Allen key, three replacement nylon socket screws for older type lenses and full instructions.
I have owned the kit for a number of years as a spare and never needed to use it so it is just as it was supplied to me by Aquatica. Cost new today would be around £170.
